Cash prices 540 hospitals

Annual physical cost without insurance

The median cash price for Annual physical, returning 18–39 is $137.00 across 540 US hospitals that publish a self-pay price. Most hospitals (the middle 80%) charge between $47.93 and $283.23.

A yearly physical is billed with a preventive visit code chosen by your age group and whether you are a new or returning patient. The listed price covers the checkup itself; lab work, vaccines and screening tests ordered at the visit are billed separately, and discussing a new or ongoing problem can add a separate office visit charge.

“Most charge” is the middle 80% of hospitals (10th to 90th percentile), so one mistyped line in a hospital file cannot stretch it. Each hospital counts once, at its lowest outpatient cash price for the code. “Medicare” is Medicare's 2026 national base rate for the same service outside a hospital stay, including the patient's share (the hospital outpatient, lab or hospital clinic rate); “median 3×” means the median cash price is three times that rate.

Paying cash: what to do

  • Ask which lab tests are usually ordered at the physical, and get their self-pay prices by code too, including the blood draw.
  • If you want a specific symptom or condition looked at, ask whether it will be billed as a separate visit, or book it as its own appointment.
  • Ask whether the clinic offers a flat cash package that includes the exam and basic labs; some clinics and direct primary care practices do.

Questions

What is included in an annual physical?

A review of your health history and medicines, a physical exam, counseling on healthy habits and ordering the screenings recommended for your age. Lab tests, vaccines and screening tests are separate services with their own prices.

Is an annual physical free with insurance?

Most private health plans cover a yearly preventive visit in network without a copay or deductible, but care for a specific problem may not be covered the same way. Medicare does not cover a routine physical; it covers a 'Welcome to Medicare' visit and a yearly Annual Wellness Visit instead.

Why was I charged for an office visit on top of my physical?

If a new problem or an existing condition is evaluated during the physical, the clinician may bill a problem-focused visit code in addition to the preventive code. Ask at the start of the visit whether a topic will lead to an extra charge.
