Lab tests, X-rays and procedures are billed on their own, and a visit at a hospital-owned clinic can add a separate hospital facility fee.

“Most charge” is the middle 80% of hospitals (10th to 90th percentile), so one mistyped line in a hospital file cannot stretch it. Each hospital counts once, at its lowest outpatient cash price for the code. “Medicare” is Medicare's 2026 national base rate for the same service outside a hospital stay, including the patient's share (the hospital outpatient, lab or hospital clinic rate); “median 3×” means the median cash price is three times that rate.

Paying cash: what to do

  • Ask whether the clinic is hospital-owned and bills a separate facility fee; independent offices usually do not.
  • Ask for the self-pay price by visit level and which level is typical for your reason for coming, since the level is set after the visit.
  • Community health centers (federally qualified health centers) charge on a sliding fee scale based on income, whether or not you have insurance.

Questions

How much is a doctor visit without insurance?

It depends on whether you are new or returning and on the visit level, which the clinician picks after the visit based on complexity or time. Tests, shots or procedures done during the visit are added, so ask what is included before you go.

Can I get a Good Faith Estimate for a doctor visit?

Yes. If you are uninsured or not using insurance, the No Surprises Act requires providers to give you a Good Faith Estimate when you schedule at least three business days ahead or when you ask for one. If the final bill is substantially higher, a federal dispute process may apply.

Why did I get two bills for one visit?

If the clinic is owned by a hospital, you may get one bill from the clinician and a second hospital facility fee for the same visit. Independent offices usually send only the clinician's bill.
