A vaccine's code is the dose itself; giving the shot is a separate administration fee (90471 for the first vaccine of a visit), so add the two lines to get a total. Vaccine prices by type

Each vaccine has its own code, and series vaccines (shingles, hepatitis, HPV, MenB) need two or three doses, so multiply the per-dose price. Flu, pneumonia and COVID-19 shots are covered by Medicare Part B, and the shingles, Tdap and RSV vaccines by Part D plans with nothing to pay.

“Most charge” is the middle 80% of hospitals (10th to 90th percentile), so one mistyped line in a hospital file cannot stretch it. Each hospital counts once, at its lowest outpatient cash price for the code. “Medicare” is Medicare's 2026 national base rate for the same service outside a hospital stay, including the patient's share (the hospital outpatient, lab or hospital clinic rate); “median 3×” means the median cash price is three times that rate.

Paying cash: what to do

  • Ask whether the self-pay price is per dose and whether it includes the injection fee (90471), then multiply by the number of doses in the series.
  • Compare with a pharmacy's cash price and your local health department, which often charge less for routine vaccines.
  • For a rabies or tetanus shot after an injury, ask whether later doses can be given at a clinic instead of the emergency department.

Questions

Where can I get vaccines without insurance?

Pharmacies post cash prices for most adult vaccines, community health centers charge on a sliding scale, and local health departments offer some vaccines free or at low cost. Children can get vaccines free through the Vaccines for Children program.

Why are there two charges for one shot?

Billing rules separate the vaccine (the product) from the work of giving it, so a bill usually has a vaccine code and an administration code such as 90471. Some hospitals combine them in one price; ask which you are being quoted.

Does Medicare cover vaccines?

Part B covers flu, pneumonia, COVID-19 and, for people at risk, hepatitis B vaccines. Part D drug plans cover other vaccines the CDC recommends for adults, such as shingles, Tdap and RSV, with nothing to pay out of pocket.
