Dental work uses CDT codes that start with D rather than CPT codes, and most of it is done in private dental offices, so the hospital prices here mainly come from hospital and dental school clinics. D7140 is a simple extraction of a visible tooth and is used in the state table; D7240 is the surgical removal of a tooth fully covered by bone, such as an impacted wisdom tooth. A deep cleaning (D4341) is priced per quadrant of the mouth, and an implant post (D6010) does not include the abutment or the crown.
