Cash or insurance?
If you have insurance but have not met your deductible, you pay the rate your plan negotiated with the hospital, not the cash price. Hospital price files list those rates too: for colonoscopy with ultrasound, the highest negotiated rate in a hospital's file has a median of $3,983.66 and the lowest a median of $617.04. At 351 of 400 hospitals that publish both, the cash price is below the highest negotiated rate.

What is CPT 45391?
This procedure combines a colonoscopy with an endoscopic ultrasound, using a special scope with an ultrasound probe on the tip. It shows the layers of the bowel wall and nearby structures, and is used to look at growths or how deep a tumor reaches.
What the price covers
The facility price covers the procedure room and recovery. The specialist's fee, anesthesia and any pathology are usually billed separately.
Why prices differ
Endoscopic ultrasound needs special equipment and trained doctors, so fewer hospitals offer it and prices vary more than for a standard colonoscopy.
What to ask
Ask whether the self-pay price for CPT 45391 includes anesthesia and the specialist's fee, and whether a needle biopsy (45392) might be done.
