Cash or insurance?

If you have insurance but have not met your deductible, you usually pay your plan's negotiated rate, which falls somewhere in the "Insurers pay" column. What it is
An amylase test measures a digestive enzyme made mainly by the pancreas and salivary glands. It is used with a lipase test for belly pain that might be pancreatitis.
