Cash or insurance?

If you have insurance but have not met your deductible, you usually pay your plan's negotiated rate, which falls somewhere in the "Insurers pay" column. What it is
This code covers giving a medicine through a vein over up to an hour, such as antibiotics, iron or some biologic drugs. Each additional hour is 96366; chemotherapy uses 96413.
What the price covers
The price is for the infusion time. The medicine is billed separately and often costs far more than the infusion.
